In the novel "Fade" by Robert Cormier, how did Bernard die?

Paul Moreaux is talking about how he is so upset that he had to deal with his cousin Bernard's death. He said that the "fade" had to do with it. I don't remember at all...How did Bernard die and why does Paul say that it's his fault???

I finished the book a while ago and don't know where to reread it to remember this detail. I have a book report coming up in a couple of days.
