need to activate new sim card,but lostthe number for it.?

i just moved house and cant find the forms,but i kept the sim in a safe place,how can i activate the sim without the number?

is there a way i could ask the ppl who activate them?
but they need the number to activate it....its vodafone btw.

am i totaly screwed?
