Protect fiance in coma/VS from mother's home care?

My fiance suffered a brainstem stroke following a brain hemorrhage. She has lesions to her midbrain, but no one is sure if/when she will wake up. It has been almost three weeks, and she is breathing on her own via a tracheostomy. She has a PEG in her stomach to provide nourishment. Her vitals are relatively stable, though her pulse is a bit high.

The hospital is looking to discharge her soon, and her mother (who is making all the decisions her my fiance) has lost touch with reality. Her mother has decided on home care after the discharge. Her mother is in her 50's, has pretty much no medical background, and lives in a rundown trailer in the middle of nowhere. The nurses are training a bit on taking care of my fiance, and apparently a hospital bed is being ordered.

But seriously, this woman doesn't know the difference between an artery and a vein. Her knowledge of my fiance's conditions and medical history is vague. There is no way she can turn her every two hours to prevent sores. The number-one killer for patients in a vegatative state is infection, especially pneumonia, and her mother has told me that she doesn't even believe alcohol can sterilize things.

I have no power of attorney. Her mother only has parental rights. This would be a death sentence for my fiance. She needs to be in a rehabilitation facility. What can I do to save my fiance from going home with her mother?
