What do I include in a suvery introduction if some people are sensitive about...

...the topic (exercise)? I would like to introduce myself as xxxxxx a grade 11 student currently studying Intro. to Psychology, Sociology and Anthropology. I am conducting a survey on levels of physical activity. Please take a few minutes of your time to read the questions thoroughly and answer the questions honestly. All information collected will be confidential and be solely for academic use.

What can I say to clarify that some questions may be personal but they should respond to the best of their ability?? And do you usually say thank you at the end of the intro or the end of the survey?
 
I would say Thank You at the end of the survey. and maybe you can tell them that it will be confidential and that it is for academic use(like you said) and just be nice about it and you might want to ask them if they feel more comportable answering the questions throught an e-mail or letter instead of person..hope this helped :)
