what does "specific medium" mean when referring to a DVD or VHS?

I'm submitting a scholarship DVD audition and I have to include an information sheet and it says :

"Include with the application and tape a video information sheet, including:
artists name
identity of each segment in order of presentation
number of segment
"specific medium"
date completed
playing time of work and/or segment

I think in your case it is : "DVD" (and you can be more explicit as in DVD-R or DVD+R
