Who: Prophet Muhammad (Peace and Blessings upon him) brought the Qur'an from Allah.
When: 610 Anno Domini / 0 Anno Hegirae How ever, most like my self, believe that Islam was the first religion, the religion of Adam.
Where: Arabia, Mecca (holiest city of Islam) & Medina.

Muhammad founded the religion of Islam. He started preaching his beliefs around 600 AD if I recall properly. A few years later he started gaining more and more followers. The religion started to bloom once Muhammad conquered Mecca at around 620 AD.

The dates could be off by several years.
