What replacement relay will work for a Ford Probe cooling fan?

I checked the cooling fan / blower relay for my 1995 4cyl 2.0L Ford Probe and it is indeed defective (it doesnt click). I have not been able to locate a proper replacement for it, just the universal stuff that isnt quite so universal. The relay has 5 horizontal connectors, in the pattern below

--- (empty) ---

The two on the left is Normally Open(NO) and the center is Com, and the other two are Always Closed I believe. Anyone know of the correct replacement part?
 
Any Ford dealership will be able to get it. Just go to the parts department.

It's worth the cost to buy if from a Ford dealer's parts counter to make sure it's designed to work in the car. Your local Ford dealer may even sell parts over the internet for a discounted price.
