Whats wrong with my starter? On my my 1951 GMC 3/4 TON?

It is the foot peg style starter, its also now a 12VT system, when the pedal down it wont always start to turn over, What normally works for me is rapidly pressing the pedal up and down till it starts to turn over.
One time I poped the hood and just pressed the button that makes it turn over and it like jumped and then a tiny bit of smoke came from it. Whats up with it?
 
Check if it's loose. Try real hard to move the starter with your hands to see if it needs to be tightened up.
