Cruise ships are designed by a combination of shipyard designers, designers who work for the cruise lines, and various specialized design firms. The job is simply a cruise ship designer. I would assume you would need a college degree in some type of interior design. Florida International University currently offers a 6-credit cruise in ship design. They'll soon expand their program by offering a 15-credit certificate in cruise ship interior design.
