The NHS Alliance, RCGP, ADASS and the National Care Forum are running a series of six workshops on "Improving Quality and Productivity in Care Homes to Reduce Hospital Admissions", during April and May. Over the last year, many reports have highlighted how the poor management of Long Term Conditions (LTCs) and medicines in care homes increases hospital admissions and out-of-hours consultations.
