It has been working fine for 3 months, then one day this message pops up and says USB device not recognized. What do I do? I tried restarting my computer switching USB ports but nothing works. Help!

I call AT&T and they gave me the PUK code but how do i put it in my phone. The thing my phone says when i turn it on it is PUK blocked contact provider.. i know you can punch in *#06# and get the IMEI # but what do i type in so i can input the PUK.?
