This is an old problem with optical drives, it occurs as the drive wears as the materials used in them are sometimes not very wear resistant.
And things move in directions they should not. then its usually time to change the drive. I have had system disks files destroyed by optical drives whilst installing an operating system.
I now know that it would be best to take an image of the operating system to restore the system when it fails and have several copies as the optic drive does not have a long life.
