i have a 1993 gmc safari mini van that everytime i turn it on and shift to "D"

it always jolts forward.? Every time i turn it on and shift from Park to Drive it jolts real strong forward then it drives forward. Anyone might know what this could be? and also when the engine is on and on park the engine started to sound alittle accelerated than before.
i forgot to mention that once i shift to "D" it sounds like a loud crank then it jolts forward.
and the same in reverse.

Does it do the same thing in Rev? if yes you need to have your U-joints checked.
