In the Harry Potter series, is Tom Riddle and "Lord" Voldemort the same person?

Im confused! In the Harry Potter and The Goblet Of Fire, when Harry and Voldemort meet up in the graveyard, Voldemort says "This is where the bones of my father lies" and the grave belonged to Tom Riddle! So hopefully you can help me! Thank you!

SPOILERS.

When Voldemort was at school, he was called ''Tom Marvolo Riddle.'' named for his muggle father and pure-blood grandfather.
His father was muggle Tom Riddle.
His mother was called Merope Gaunt, and her father is Marvolo Gaunt.

When Riddle left school he killed his muggle father, and scrapped his muggle name.
''TOM MARVOLO RIDDLE'' id an anagram of ''I AM LORD VOLDEMORT.''

Edit: Half Blood Prince explains a lot about Voldemorts past.

yes they are the same person. Tom Marvolo Riddle. Rearrange them and you get Lord Voldemort

Tom Riddle was Lord Voldemort as a young guy. The grave was of his father, who had the same name.

yes Tom Riddle was his real name... he decides to come up with a name that people will fear to speak.... in the second movie it shows "Tom Riddle" explaining it... "Tom Marvolo Riddle" has the same letters as "I am Lord Voldemort"
