Where can I find a 2-disc DVD case that still has insert clips on the left side?

I need a DVD case that holds two discs but still have the clips inside to put a small booklet; the kind with the disc flap and the normal disc spot. Everything I've looked at has either been 1 disc on each side or has had the flap but been for 4 or more discs.

Price does not matter, any help is appreciated.
